In the event that my child is hospitalized or treated medically causing extended absence, what is the process for returning to school?
Upon hospitalization, please contact your child's Guidance Counselor as soon as possible to discuss a plan for returning to school. Guidance staff will connect with RISE, and discussion will begin regarding possible program participation. RISE will reach out to you, and will discuss options, as well as the re-entry process when they are ready to return to school.

Whom can I speak with further about the RISE Transition Program?
School personnel, including the student's teachers, Counselors, Adjustment Counselor, Nursing staff, and              Administration may be made aware of participation in the RISE Transition Program. However, details regarding the    student's absence from school are kept confidential. We are very serious about confidentiality, and understand that your privacy is paramount.
If you have questions about the program, please contact: Dave Worthley, Ed.D., Coordinator of Social/Emotional Learning at Medfield High School, (508) 242 - 8548.
